Documents to Gather
- ☐ Photo identification (driver's license or state ID)
- ☐ Insurance information (Medicare, Medicaid, supplemental, long-term care)
- ☐ Current medication list (names, dosages, frequency, prescribing physician)
- ☐ Medical records and recent health summary from primary care physician
- ☐ Advanced directives (living will, healthcare power of attorney, DNR if applicable)
- ☐ Emergency contact information for family members and physicians
- ☐ Pharmacy contact information
- ☐ Social Security card or documentation
- ☐ Financial and payment information
Personal Items to Bring
Clothing & Linens
- ☐ Comfortable everyday clothing (labeled with resident's name)
- ☐ Seasonal outerwear (coat, sweater, jacket)
- ☐ Comfortable shoes with non-slip soles
- ☐ Slippers and robe
- ☐ Personal bed linens if desired (we provide linens, but you're welcome to bring your own)
- ☐ Towels and washcloths if preferred
Personal Care Items
- ☐ Toiletries (shampoo, soap, toothbrush, toothpaste, deodorant, etc.)
- ☐ Glasses, hearing aids, dentures (and cases/cleaning supplies)
- ☐ Hair care items (brush, comb, styling products)
- ☐ Shaving supplies if needed
- ☐ Preferred skin lotions or ointments
Room & Comfort Items
- ☐ Favorite photos and picture frames
- ☐ Small furniture if desired (chair, lamp, nightstand—check with staff on available space)
- ☐ Blanket, quilt, or throw
- ☐ Clock and calendar
- ☐ Books, magazines, or e-reader
- ☐ Hobby supplies (puzzles, crafts, music, etc.)
- ☐ Television (optional; cable available)
- ☐ Phone or tablet for staying connected with family
What NOT to Bring
- ☐ Medications (we will coordinate with your pharmacy for secure storage and management)
- ☐ Large valuables or irreplaceable items
- ☐ Space heaters or other electrical appliances that pose safety risks
- ☐ Perishable foods (meals and snacks are provided)
Before Move-In Day
- ☐ Schedule a pre-move-in meeting with our care coordinator
- ☐ Complete and sign all admission paperwork
- ☐ Arrange for prescription transfers to our partnering pharmacy (we'll help with this)
- ☐ Label all clothing and personal belongings with resident's name
- ☐ Inform family and friends of the move and new address
- ☐ Set up mail forwarding if necessary
- ☐ Update contact information with healthcare providers
Move-In Day
- ☐ Arrive at agreed-upon time (we'll help coordinate)
- ☐ Meet staff and take a tour of the home with your loved one
- ☐ Unpack and personalize the bedroom
- ☐ Review care plan and daily routine with care coordinator
- ☐ Ask any last-minute questions
- ☐ Say hello to other residents and make introductions
After Move-In
- ☐ Plan regular visits and phone calls
- ☐ Attend the first care conference (typically within 30 days)
- ☐ Communicate openly with staff about preferences, concerns, and feedback
- ☐ Participate in family events and activities when invited
